While abdominal distention (the \u201cpregnant belly\u201d) is often assumed to indicate SIBO, accurate diagnosis requires careful differential assessment to avoid unnecessary antibiotic use \u201c<\/p>\n<hr data-start=\"2670\" data-end=\"2673\" \/>\n<p data-start=\"24693\" data-end=\"24712\">\n<p>[\/et_pb_text][et_pb_text module_id=&#8221;defining&#8221; _builder_version=&#8221;4.27.4&#8243; _module_preset=&#8221;default&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<h2 data-start=\"2675\" data-end=\"2731\">Defining Small Intestinal Bacterial Overgrowth (SIBO)<\/h2>\n<p data-start=\"2733\" data-end=\"3042\">Small intestinal overgrowth (SIBO) is clinically defined as the presence of an abnormal bacterial load\/dysbiosis in the small intestine (1).<br data-start=\"2873\" data-end=\"2876\" \/>Unlike infections caused by a single pathogen, such as H. pylori, SIBO represents a <strong>quantitative shift in the commensal gut microbiota<\/strong> (Non-pathogenic microorganism).<\/p>\n<p data-start=\"3044\" data-end=\"3403\">Specifically, we see an overgrowth of Klebsiella and Escherichia coli (2), species usually resident in the colon VS the small intestine (3). These strains are directly linked to the severity of bloating, pain, and diarrhea (2) . Altough, contrarly of what was tought before, it is NOT the bacteria of colon moving in the small intestine like tought before (2)<\/p>\n<p data-start=\"3405\" data-end=\"3807\">Under normal physiological conditions, bacterial density in the upper small intestine is strictly regulated by the antimicrobial effects of gastric acid and the mechanical clearing action of peristalsis (1).<br data-start=\"3612\" data-end=\"3615\" \/>In SIBO, however, these <strong>physiological defenses become compromised<\/strong>; consequently, <strong>bacterial colonization exceeds physiological norms<\/strong>, directly <strong>driving<\/strong> the development of <strong>digestive distress<\/strong> (3).<\/p>\n<p data-start=\"3809\" data-end=\"4246\">Crucially, SIBO must be understood not as a primary disease, but as a <strong>secondary consequence<\/strong> of an <strong>underlying dysfunction<\/strong>. It is a clinical manifestation of a deeper physiological or mechanical failure.<br data-start=\"4010\" data-end=\"4013\" \/>Consequently, <strong>any treatment that focuses solely on eradicating the bacteria overgrowth will fail in the long term<\/strong>; the overgrowth will inevitably return as long as the primary physiological problem (\u201croot cause\u201d) remains unaddressed.<\/p>\n<p data-start=\"4248\" data-end=\"4654\">Later, we will differentiate SIBO from distinct entities like IMO (Intestinal Methanogen Overgrowth) and ISO (Intestinal sulfide overproduction). The drop in B12 is largely due to the destruction of binding sites within the ileum, caused by localized mucosal damage from bacterial overgrowth (1) (3)<\/p>\n<hr data-start=\"6975\" data-end=\"6978\" \/>\n<h2 data-start=\"6980\" data-end=\"7004\">Note on IMO and ISO :<\/h2>\n<p data-start=\"7006\" data-end=\"7214\">The symptomatology of IMO (Intestinal Methanogen Overgrowth) and ISO (Intestinal Sulfide Overproduction) is distinct, primarily driven by the metabolic effects of their respective gases on intestinal transit.<\/p>\n<p data-start=\"7216\" data-end=\"7390\"><strong>Intestinal Methanogen Overgrowth<\/strong> (IMO) is characterized by <strong>excessive methane<\/strong> (CH4) production, which is strongly correlated with <strong>delayed transit<\/strong> and <strong>chronic<\/strong> <strong>constipation<\/strong> (6).<\/p>\n<p data-start=\"7392\" data-end=\"7560\">Conversely, Intestinal Sulfide Overproduction (ISO) is associated with elevated hydrogen sulfide (H2S), which typically manifests as increased urgency and diarrhea (11)<\/p>\n<h3 data-start=\"7562\" data-end=\"7602\">Clinical observations from practice:<\/h3>\n<p data-start=\"7604\" data-end=\"7940\">While less documented in current scientific literature, my clinical experience reveals that many patients also report <strong>joint pain<\/strong> and <strong>acid reflux<\/strong>. The reflux, in particular, is likely a secondary mechanical consequence of severe abdominal distention, which increases intra-abdominal pressure and displaces the lower esophageal sphincter.<\/p>\n<p data-start=\"7942\" data-end=\"8114\">Clinical management should prioritize investigating the underlying etiology of reflux rather than defaulting to the long-term prescription of Proton Pump Inhibitors (PPIs).<\/p>\n<hr data-start=\"8116\" data-end=\"8119\" \/>\n<p data-start=\"24693\" data-end=\"24712\">\n<p>[\/et_pb_text][et_pb_text module_id=&#8221;diagnostic&#8221; _builder_version=&#8221;4.27.4&#8243; _module_preset=&#8221;default&#8221; global_colors_info=&#8221;{}&#8221;]<\/p>\n<h2 data-start=\"8121\" data-end=\"8208\">The SIBO &#8211; IMO Diagnostic Journey: Understanding Breath Tests and Clinical Realities<\/h2>\n<p data-start=\"8210\" data-end=\"8360\">In the absence of a validated diagnostic gold standard, SIBO is usually identified via breath tests or jejunal aspirates (1). Both have Pros and Cons.<\/p>\n<ul data-start=\"8362\" data-end=\"8973\">\n<li data-start=\"8362\" data-end=\"8512\">\n<p data-start=\"8364\" data-end=\"8512\"><strong data-start=\"8364\" data-end=\"8387\">Jejunal aspiration:<\/strong><br data-start=\"8387\" data-end=\"8390\" \/>Despite its validated diagnostic threshold of &gt;10^3 CFU\/mL, jejunal aspiration is rarely utilized due to its invasiveness.<\/p>\n<\/li>\n<li data-start=\"8514\" data-end=\"8973\">\n<p data-start=\"8516\" data-end=\"8973\"><strong data-start=\"8516\" data-end=\"8532\">Breath test:<\/strong><br data-start=\"8532\" data-end=\"8535\" \/>The theory behind breath testing is that human cells do not produce hydrogen (H2) or methane (CH-4) gases.<br data-start=\"8641\" data-end=\"8644\" \/>Their presence in a breath sample is a then a sign of microbial fermentation in the gut. When bacteria break down carbohydrates, they release these gases, which are absorbed into the bloodstream and exhaled through the lungs. This allows for a simple, non-invasive way to measure what is happening inside the small intestine. (3)<\/p>\n<\/li>\n<\/ul>\n<p data-start=\"8975\" data-end=\"9275\">The American College of Gastroenterology recommends 75g of <strong>glucose<\/strong> or 10g of <strong>lactulose<\/strong>, with a cup (250ml) of water (3).<br \/>They consider that a rise in hydrogen concentrations of <strong>&gt;20 ppm from baseline, within a specific window<\/strong> to avoid capturing colonic fermentation, is considered <strong>diagnostic for SIBO.<\/strong><\/p>\n<p data-start=\"9277\" data-end=\"9432\">On the other hand, the presence of methane concentrations <strong>&gt;10 ppm at any point during the test<\/strong> is diagnostic of <strong>Intestinal Methanogen Overgrowth<\/strong> (IMO) (3).<\/p>\n<p data-start=\"9434\" data-end=\"9634\">It is not uncommon for patients to test positive for both hydrogen-producing bacteria and methane-producing archaea simultaneously, a finding that typically warrants a more complex treatment approach.<\/p>\n<h3 data-start=\"9636\" data-end=\"9645\">Note:<\/h3>\n<p data-start=\"9646\" data-end=\"9961\">Rigorous preparation is essential prior to a breath test to prevent false positives or negatives. This includes a specific preparatory diet and the strict avoidance of certain supplements and medications (when possible) for at least two weeks before the procedure. Those should not be attempted without supervision.<br \/>Also, a Low-Fodmap Diet should NOT be followed before a breath-test.<\/p>\n<hr data-start=\"10469\" data-end=\"10472\" \/>\n<h2 data-start=\"10474\" data-end=\"10513\">Are breath tests for SIBO reliable ?<\/h2>\n<h3 data-start=\"10515\" data-end=\"10608\">Scientific Validation of Lactulose Hydrogen Breath Testing (LHBT) (recent data from 2025)<\/h3>\n<p>&nbsp;<\/p>\n<p data-start=\"10610\" data-end=\"10862\"><strong data-start=\"10610\" data-end=\"10635\">Microbial Validation:<\/strong> Breath testing has been validated against small bowel culture and sequencing. Hydrogen levels detected on the test correlate with both patient-reported gas symptoms and the metabolic activity of the small bowel microbiota (7).<\/p>\n<p data-start=\"10864\" data-end=\"11182\"><strong data-start=\"10864\" data-end=\"10892\">Transit Differentiation:<\/strong> Using scintigraphy, research <strong>confirms that intestinal transit time does not explain a positive breath test<\/strong>. A hydrogen peak of &gt; 20 ppm before the 80-minute mark is a <strong>valid indicator of SIBO<\/strong>, allowing clinicians to effectively sub-divide IBS patients into SIBO and non-SIBO phenotypes (8).<\/p>\n<h3 data-start=\"11184\" data-end=\"11233\">Debates between Glucose or lactulose testing:<\/h3>\n<p data-start=\"11235\" data-end=\"11577\">Dr. Mark Pimentel, a preeminent researcher in the field, advocates for the use of lactulose as the preferred substrate for breath testing.
